Bridging the Gap to Home

Our rehab team works with you or your loved one to reach your health goals as quickly and efficiently as possible. We partner with physical, occupational and speech therapists to achieve these aspirations of independence. We provide equipment and highly trained staff to achieve the balance of making this possible. Windsor’s ultimate goal is to safely return residents to their homes, or other appropriate living arrangements, as soon as possible. While each resident’s therapy regimen is prescribed by their physicians, progress towards goals is assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded. Once rehabilitation goals have been achieved, discharge planning occurs including home assessments and in-depth caregiver training to support our resident’s continued success in their own environment.

In addition to long-term skilled nursing care, we also provide short-term rehabilitation care after a surgery or traumatic medical experiences, such as a stroke or heart attack, and long-term care for those with more complex needs. Our spacious therapy center features all of the necessary equipment for practicing and relearning vital skills, while our team of fully certified therapists can develop or modify programs according to individual abilities and limitations.

SHORT-TERM REHABILITATION

Our short-term rehab services can help bridge-the-gap between the hospital and home and allow you to cultivate the necessary skills to lead an independent lifestyle. Our short-term rehab program provides 24-hour skilled nursing care and a full range of rehabilitative services—respiratory therapy, intravenous therapy, physical therapy, occupational therapy and speech therapy. During your stay, the therapists will have you practice, perform, and perfect all the skills you need before being discharged.

LONG-TERM CARE

We also provide medically-supervised long-term care for those who require long-term, 24-hour care by a team of nurses and therapists. The most common needs of long-term care residents at skilled nursing centers include daily medical management and assistance with activities of daily living. For some residents, we are able to restore them to a level of function which enables them to return home. For those who cannot return home, we are committed to providing a warm and caring home and feel honored they chose to make our community their home.

OUTPATIENT REHABILITATION

Outpatient rehabilitation is used for people who are living safely at home but require rehab therapy to help them live free of physical limitation or pain. Outpatient therapy programs are for those who no longer need inpatient therapy but may benefit from a planned schedule of therapy. Outpatients have access to our inpatient therapy space and equipment scheduled around their daily lives.
